It's an Estonian "Lōõtspill" or diatonic "harmonica" or "accordion".
Frequently, the player sings the song while playing the tune on the lõōtspill.
Much adopted/adapted, recognisable popular music of other European countries is also played on the instrument.

August Teppo was first who built these kind of instruments in Estonia, this is reason why it is called Teppo type diatonic - after first builder.
